Care Coordinators are responsible for supporting individuals with disabilities, by providing clinical management and oversight of the professionals who work with them, as well as building a relationship and overseeing the families and individuals on their caseload. Up to 25% travel may be required.

SUMMARY OF RESPONSIBILITIES
* Expediently fill referrals with appropriately matched professionals, to meet and increase established billable hours
* Create and maintain clinically appropriate goals and objectives for clients
* Review staff's monthly and quarterly notes for completion, and update as necessary
* Ensure complete events in database for all calls and business made on a daily basis
* Resolve service problems by answering complaints and inquires, clarifying the complaints, determining the cause of the challenge, and conducting follow-up to ensure resolution
* Attend quarterly, yearly, or as-needed meetings for all clients on caseload

REQUIRED EXPERIENCE
* Minimum of one (1) year experience working within the field of human services, case management, or a related field

SALARY
Compensation is commensurate with experience
